Active Care Plans

• 12 session care plan - Pain Relief & Restorative/Stabilizing Phases
• $900 (2 installments of $450 or 3 installments of $300) / $780 if paid in full 
• Included: 
• 2 sessions/week for 6 weeks (typical) OR 3 sessions/week for 2 weeks; 2 sessions/week for 3 weeks (severe) OR 1 session/week for 12 weeks - customized w/ Dr. Charm depending on pain levels, scheduling and finances
• Cupping or Muscle Work
• Re-Evaluation at visit 12 ($150 value)

Maintenance Care Plans

• Option 1: $150/month 
• Includes:
• 2 sessions / month

• Re-Evaluation every 12 visits (2/year)
• Option 2: $75/month
• Includes: 
• 1 session/month

• Re-Evaluation every 12 visits (1/year)

Bulk Adjustments:
